π What is Dengue Fever?
Dengue fever is a mosquito-borne viral disease caused by any of four dengue virus serotypes (DENV-1, 2, 3, 4) transmitted by the Aedes aegypti mosquito β which bites primarily during the day. India is among the countries with the highest dengue burden globally β with millions of cases annually and seasonal surges during and after the monsoon (JulyβNovember). Dengue ranges from a mild self-limiting fever to severe dengue (dengue hemorrhagic fever/dengue shock syndrome) which can be fatal if not managed promptly. There is currently no specific antiviral treatment β management is supportive, making early recognition of warning signs critical. India's rapid urbanization, monsoon breeding conditions, and dense population make dengue a major public health challenge.
π Dengue Severity Classification
| Category | Features | Management |
|---|---|---|
| Dengue Without Warning Signs | Fever + 2 of: nausea/vomiting, rash, aches/pains, positive tourniquet test, leukopenia | Outpatient β home management with close monitoring |
| Dengue With Warning Signs | Abdominal pain, persistent vomiting, fluid accumulation, mucosal bleeding, lethargy, liver enlargement, rising hematocrit with rapid platelet fall | Hospital admission for IV fluid management |
| Severe Dengue | Severe plasma leakage (shock), severe bleeding, severe organ impairment (liver, CNS, heart, kidneys) | ICU admission β emergency |
β οΈ Dengue Warning Signs β Go to Hospital Immediately
After the initial fever subsides (day 3β7), watch carefully for these warning signs that indicate severe dengue: Severe abdominal pain or tenderness, persistent vomiting (3 or more times in 1 hour), bleeding from gums, nose, or in vomit/stool/urine, blood in urine (tea-colored or pink), extreme tiredness, restlessness or extreme irritability, and rapid breathing with cold/clammy skin (shock). These signs require immediate hospital evaluation β delay can be fatal.
π©Ί Diagnosis & Platelet Count
Diagnosis is confirmed by NS1 antigen test (days 1β5 of fever), Dengue IgM antibody test (from day 5), or PCR. Complete Blood Count (CBC) is essential β dengue typically causes thrombocytopenia (low platelets) and leukopenia (low white blood cells). Monitor platelet count daily during illness. Normal platelet count: 1.5β4 lakh/mmΒ³. Hospital admission recommended when platelets fall below 50,000/mmΒ³ with symptoms; blood products (platelet transfusion) considered below 10,000/mmΒ³ or with significant bleeding.
π Supportive Treatment
Hydration: The cornerstone of dengue treatment. Plasma leakage (fluid leaving blood vessels) causes shock β aggressive oral or IV hydration maintains blood pressure. Drink 2β3 liters of ORS, coconut water, fruit juices, and water daily. Paracetamol only for fever β NEVER aspirin, ibuprofen, or diclofenac (these worsen bleeding risk by affecting platelet function). Rest: Complete bed rest during febrile phase. Monitoring: Daily CBC, platelet count, and hematocrit during critical phase (days 3β7).
πΏ Natural Platelet Boosting Remedies
| Remedy | Preparation | Evidence/Benefit |
|---|---|---|
| Papaya Leaf Extract | Crush 2β3 fresh papaya leaves, extract juice (about 30ml), take 2x daily; or commercial capsules | Multiple Indian clinical trials confirm papaya leaf significantly increases platelet count in dengue |
| Giloy (Guduchi) Juice | 30ml fresh giloy stem juice or decoction twice daily | Immunomodulator β reduces fever duration, supports recovery; clinical evidence in dengue |
| Pomegranate Juice | Fresh pomegranate juice, 1β2 glasses daily | Iron, antioxidants β supports blood health; traditional platelet support remedy |
| Kiwi Fruit | 2β3 kiwis daily | Vitamin C, Vitamin K β supports platelet production and function |
| Coconut Water | 2β3 coconuts daily | Electrolyte replacement β prevents dehydration and shock; traditional dengue hydration |
| Amla (Indian Gooseberry) | Fresh amla juice or 2β3 raw amla daily | Highest Vitamin C β antioxidant, immune support; aids recovery |
π‘ Dengue Prevention β Mosquito Control
Eliminate stagnant water weekly β check coolers, flower pots, buckets, old tyres, drains. Use mosquito repellents with DEET or Picaridin (more effective than natural alternatives for dengue prevention). Wear full-sleeved clothing during day, especially at dawn and dusk. Use bed nets (even during day β Aedes bites during daytime). Install window and door screens. Report suspected dengue clusters to your local municipal corporation β they can arrange fogging. Dengue Vaccine (Dengvaxia) is available in India β consult a doctor for eligibility (only for those with previous dengue confirmed by blood test).
